Do you now, or did you, work in the United States or a territory or possession of the United States in the last three years?
In the last three years have you worked more than forty hours in a work week without receiving legally entitled overtime pay for the hours over forty?
Here are some examples of situations in which overtime pay may be due (If any of these scenarios apply to you, please note them in the box above): 1) We normally only worked forty hours per week, but when a big project was due, my manager told us to clock out and keep working without being paid for those extra hours. 2) After I put on my required protective gear at the start of my shift, I spent 20 minutes a day or longer of unpaid time walking from the locker room to my workstation. 3) After the new supervisor came, I was required to work 8 am to 7 pm every Monday to Friday, 55 hours a week, but I was only paid for 40 hours. 4) I was required to report to work 30 minutes early for a pre-shift briefing, but wasn't paid for that time. 5) I was required to stay after my shift ended to brief the incoming shift but wasn't paid for that time. 6) After my shift ended, I wasn’t paid for the time it took me to walk back from my workstation to the locker room and take off my required protective gear. 7) My employer told me I am a salaried exempt manager not entitled to overtime pay, but I do not supervise two or more full-time employees. 8) My employer’s time-keeping system did not record some of my working time and I was only paid for time the system recorded.
